SOAR Family Intake Form (Special Needs Ministry)

Rocky Mount Church is very excited about our new Special Needs Ministry called SOAR. Our vision is to transform religion into a relationship with Jesus Christ, one person at a time, by providing SUNDAY School classes, ONE-ON-ONE buddies, and seasonal ACTIVITIES to individuals with I/DD, so they and their families can build RELATIONSHIPS within the church community.

We have a class at 11:00 on Sunday mornings and we would love to have you join us but we ask that you fill out this form for the person that will be attending this class first. Once your form is received, you will receive an email from soar@rockymountchurch.com with more information.
